If in the preRBL module you have selected only 1 RBL you should not put the "List hits to be spam" at 3 !!! because this 3 lists will never be reach ... place it to 1. this selector is to decide how many list should detect the message before considering the message as spam, it make no sense to configure it at a value bigger than the number of active lists.
1) if you have a gateway before MailCleaner you need to trust it, because all the IP and reputation test are done for the previous IP in the mail Header, assuming this is the IP of the sender server. But if you have a gateway in front of MailCleaner you need to trust it in the config to apply all the test to the previous IP ... If not you will have very bad filtering quality ...
2) have a look at the points in the header of the message and remember that with the community Edition you have to train the product by yourself and modify the rules by yourself ...
